Utah Parents United believes parents should have control over their family's personal information, including vaccine records stored in the Utah Statewide Immunization Information System (USIIS). USIIS is a centralized, computerized database that allows health insurers, schools, child care programs, publicly funded programs, and personal health record systems to access immunization records. While participation in USIIS is automatic, individuals or guardians can choose to withdraw (opt out) if they prefer.

​Families may opt out for reasons such as privacy concerns, wanting to manage their own medical records, limiting access to sensitive information, concerns about data security, or personal beliefs about health tracking.

If you decide to withdraw, it's recommended to take screenshots of your records in the app for your personal files before they are removed.

Withdrawal: In Utah, the process to withdraw from the Utah Statewide Immunization Information System (USIIS) is outlined in Utah Administrative Code R386-800-6.

Follow these steps to withdraw:
  1. Submit a withdrawal form, which can be found on the Utah Department of Health's website. The form should be completed and returned via email, mail, or fax. Link to form
  2. Allow 2-4 business day to process. You should receive a confirmation email or letter which will confirm your withdrawal from the system.
  3. It is also a good idea to check the docket app again to see if any of your information pops up. If you see nothing that means the withdrawal was successful.
Once the form is processed, the individual's immunization records will be wiped from the system and no longer shared with healthcare providers, schools, or other authorized entities.

Utah Immunization Exemption Law Update
The law has changed in Utah!

Parents can claim an exemption from immunization for medical, religious, or personal reasons under Utah Code § 53G-9-303. You can also claim an exemption if you choose not to provide your child’s vaccine information to the school.
Key Updates (HB 228 – Public Education Immunization Amendments)
  • One-time opt-out – no more annual renewal. 
  • Covers K–12 as long as your child remains enrolled in public school.
  • Stays with school records if your child transfers to another school in the same district.
  • Protected in Utah law – confirmed by the Utah Department of Health.

How to Claim an Exemption
Option 1: Online Immunization Education Module
  1. The legally responsible individual completes the online education module to receive the exemption form.
  2. Upon completion, download the vaccination exemption form.
Option 2: In-Person Consultation (Utah Code § 53G-9-304)
  1. Visit your local health department.
  2. Receive an in-person consultation and obtain the exemption form.

Final Step:
Submit your exemption form to your child’s school.

How Long Is It Valid?
  • One-time opt out
  • Applies for the entire time your child attends K–12 in Utah public schools.
  • If your child transfers within the district, the exemption form follows their school records.​
